The PLATINUM HEALTH ABC prepaid package is a modern healthcare solution dedicated to people of all ages who are searching for a flexible healthcare model as well as appreciate their time and health.

By making a prepayment of PLN 2,500 under the PLATINUM package, the patient may benefit from the services available at Damian Medical Centres with an attractive 15% discount.

Once purchased, the package may be used by any number of individuals, regardless of their family relationship, age or health. All the buyer has to do is to register the selected individuals upon purchasing the package or throughout its validity.It is a product designed for families, seniors, children and pregnant women

The scope of the PLATINUM HEALTH ABC prepaid package:

value of the package: PLN 2,500 discount on the medical services covered by the package: 15% validity period: 18 months discount on services not included in the package: 5% The PLATINUM HEALTH ABC prepaid package includes: medical consultations (including consultations with professors), diagnostic tests - including ultrasound, KTG, Holter, stress test, spirometry, audiometry, ECG, EEG, EMG, videolaryngoscopy, laboratory tests, rehabilitation (apart from whole body relaxation massage, partial relaxation massage, gene¬ral condition exercises, at-home rehabilitation, physiotherapist consultation – pelvic floor muscle therapy), outpatient procedures, skin and patch allergy tests, desensitisation.

We also offer an additional 5% discount on:

• endoscopic examinations (gastroscopy, colonoscopy, rectoscopy, sigmoidoscopy),
• histopathological examinations,
• genetic testing, highly specialized ultrasound, 
• preventive vaccinations 
• treatments and surgeries at Damian Hospital
• Payment for the above services is made outside the package.

Services included in the package without discount:

• psychology, psychiatry and speech therapy services
• services at the retina clinic
• botulinum toxin treatment
• minor surgical procedures for medical indications
• outpatient cystoscopy
• esperal implantation
• home rehabilitation
• consultation at a medical marijuana treatment center
• gynecological cryotherapy
• Pappa test

The package does not include medical services in the field of:

• aesthetic gynecology,
• aesthetic medicine,
• radiological examinations (X-ray, mammography, computed tomography, magnetic resonance imaging),
• genetic testing
• part of rehabilitation services (partial relaxation massage, full-body relaxation massage, general conditioning exercises, physiotherapist consultation - pelvic floor muscle therapy),
• delivery of hyaluronic acid or plasma,
• surgery and hospitalization,
• services performed outside the outpatient clinics of the Damian Medical Center,
• services and medical goods subject to VAT at the rate of 23% or 8%.

When is it worth using the HEALTH ABC prepaid package?

If you plan to use or are already using medical services in the area of:

• medical consultations for children and adults (the packages include all available specialists),
• diagnostic and laboratory tests,
• rehabilitation for adults and children.

The buyer may start using the prepaid package in three working days after paying the purchase price.

The prepaid package may be implemented only in Warsaw, Piaseczno and Szczecin.

The buyer may indicate other persons who will be entitled to use the package:

• at Damian Medical Centre, by submitting the signed application with indication of the package value, purchase date, as well as the name, surname, PESEL No. (or the date of birth) and address of the Buyer or the person removed from or included into the package,
• or by sending a scan of such an application to dak@damian.pl.

In the case of a child without a PESEL number, it is necessary to provide his/her parent’s PESEL number, while in the case of a foreign national without a PESEL number – the passport number.

A specimen of the application can be found in an appendix to the General Terms and Conditions for Selling the Healthcare Packages of Damian Medical Centre in Medistore. It can be obtained at the reception desk of any Damian Medical Centre or downloaded from: www.damian.pl.

A change in the persons authorised to use the package takes effect within twenty four hours after Damian Medical Centre has received a complete application. The applications are processed on working days.
